From a jiu-jitsu vascular-neck-restraint perspective, prolonged knee pressure on the back of the neck can indirectly compress the carotid by flushing the front of the neck against the ground, so it is absolutely possible the knee on the neck caused enough blood-flow obstruction to lead to unconsciousness and death.
