Observational epidemiology can establish causality only when effects are enormous and no alternative hypothesis is plausible — like the roughly 20-fold increased lung cancer risk in smokers — whereas the diet-disease effects on which public health policy is based are small (20% or 50% increased risk), for which countless confounding explanations are imaginable.
