Organizational culture model distinguishing maturity levels of failure tolerance: Level 1 (failure = career death), Level 2 (claim to tolerate but really don't), Level 3 (tolerate in innovation sandboxes but not core), Level 4 (measured and pervasive). Level 4 organizations ask teams quarterly: how many experiments ran, how many succeeded, how many failed? (expecting failures.)
